To schedule a New Patient appointment please fill out the following

If you would like to e-mail admin@activechiro.org your Name, Phone Number, Date of Birth and a good time to call you back we will be more than happy to do so. Same day appointments can be made, however, e-mailing a copy (front and back) of your Driver’s License and Insurance Card will expedite insurance verification. Online scheduling is not available at this time through this website (your privacy is our priority).

What to expect:

New patients will undergo a detailed history of current illness, thorough examination and review of findings. Treatment will be given at first visit if medically appropriate AND “homework” will be recommended for a faster recovery and better outcome.

Imaging:

X-rays can be ordered through Iredell Memorial Hospital the same day or at your convenience following the initial exam. MRI’s and other imaging may be ordered through Novant Imaging Services or through Iredell Memorial Hospital.
